Having a 100 hour massage course certification allowed me to connect and understand the layering of muscles better. Therefore, when I took the 3 day seminar I was able to excel at Active Release Technique. The education and experience I received at the course was mind blowing. How was I so fortunate to learn so many incredible protocols for fixing upper extremity conditions. The course was the first major step in my journey to becoming a North Vancouver Chiropractor.

Along my journey there has been many people who have had a misconception regarding Chiropractic care. My good friend in California was a Silicon Valley executive with a PH.D from Stanford, who always had some doubt about Chiropractors ability to create results. It was evident after visiting them this year and them requesting my shoulder protocol to improve and restore range of motion to their shoulders. They told me they kept asking their Physical Therapist at a prominent hospital to do the same technique I did, as the physical therapist was not able to help their shoulder. What is all the fuss about instrument assisted soft tissue mobilization (IASTM) technique. Just recently I signed up for Smart Tools plus seminar in Portland, Oregon. I purchased a set of smart tools as well as enrolling in a two-day seminar. Before the re emergence of IASTM, techniques such as active release technique were dominating the chiropractic, registered massage therapists and physical therapist seminar circuit. Active release technique, which I have attended for their upper extremity seminar is an advanced scar tissue release technique that is designed to deal with nerve impingement, repetitive trauma injuries, pain syndromes and old injuries. There are many other techniques that address muscle injury and nerve impingement syndromes.

As a North Vancouver Chiropractor, I have had excellent results using active release technique on shoulders, the upper thoracic spine and the cervical spine. The most common disorder I have addressed is rotator cuff tendonitis. As a North Vancouver Chiropractor I always muscle test each specific muscle and look for pain and weakness. Once the isolated muscle is identified I would perform active release technique. The only draw back with using my hands as a muscle reparative system is that the stress on the thumbs and hands can cause arthritis in the long term.

SMART TOOLS PLUS

Smart Tools Plus is different than other instrument assisted soft tissue mobilization, as it is not modeled solely on the Chinese practice of Gua Sha. Other IASTM tools promote bruising of the skin. In order to understand why bruising is wanted we must go back to Gua Sha. The name Gua Sha translates into scraping in order to lightly bruise the skin in Chinese. Scraping is said to promote healing by causing an inflammatory response to remove scar tissue. Smart Tools Plus does not promote bruising even though it may occur. Instead we are taught to focus working on the tissue affected with the specific tool and also using active range of motion with the tool to remove adhesions and also lengthen fascia.

Instrument assisted soft tissue mobilization or IASTM is affective for many conditions. IASTM is beneficial for Patients suffering from repetitive cumulative trauma, sports injuries, automobile accidents, lifting injuries, muscles spasms from emotional stress, and other traumas.

As a North Vancouver Chiropractor, using ergonomic surgical stainless steel tools will allow me to prevent having stress on my hands from treatment. There is more leverage or pressure per square inch with Smart Tools in the low back muscles such as multifidi and quadratus lumborum than using your hands or thumbs. From my experience I would recommend starting off with 3-4 treatments for most scar tissue adhesion syndromes where pain and limited range of motion are present. Treatments should continue as long as range of motion is increasing and pain is decreasing with each proceeding treatment. After the first treatment pain and bruising are likely to occur. As with working out for the first time in a long while the bodies tissues will become sore due to load and stretch. Thereafter, the body should have tolerance to the treatment. Therefore, soreness should subside after the second treatment. It is also very likely that that no soreness occurs after a treatment. Every body and every person’s pain tolerance will vary so it is not a one size fits all theory.

According to Wikipedia.org’s page on whiplash, during acceleration deceleration accident the thoracic spine is put under tremendous pressure and is subjected to axial load. This in turn transfers massive load to the lumbar and cervical spine. However, the thoracic and lumbar spine due there position is protected from large movements. The cervical spine does however undergo major trauma. The cervical spine reverses its normal curvature and creates a military curve or reversed curve. As the front of the cervical spine is pushed back and forth masterfully fast the anterior longitudinal ligament becomes traumatized. The micro trauma causes adhesions to build up and the ligament tightens to cause change in the curvature to the cervical spine. A normal spine should have a lordosis throughout the cervical spine. This in conjunction with the rest of the spine balances the stress on the spine and allows the nervous system to function properly. As the cervical spine reverses its course a person will start to experience pain in the back of the neck, shoulders, numbness tingling in the hands, and headaches to name a few. In terms of physical changes the person will develop spondylosis to the cervical spine, degenerative disc disease and nerve compression. The changes generally occur at C5, which is the apex of the cervical curve.

What is the prognosis of a whiplash patient? The prognosis is moderate to poor, and long-term dependence on medications, neurological deficits, and surgery are highly common. As a rule, soft tissue injuries, specifically sprain/strain injuries readily heal. However, they heal with scar tissue. Current literature notes that this alteration in normal tissue subsequent to healing of sprain/strain injuries is the fibrosis of repair”. This fibrotic repair tissue will exhibit three things:

  1. It is weaker tissue
  2. It is less elastic
  3. There is an increase in the anatomical number of nerves fibers into this healing tissue, which makes them more sensitive to future stresses and strains.

Therefore, people so affected by the healing residual of soft tissue injuries will frequently experience symptomatic exacerbations secondary to the use of the once injured parts. It should be noted that it is likely that any activity that causes increased stresses on the once injured ligaments and muscles of the cervical spine will create a symptomatic exacerbation of the patient’s condition.

What Is Scar Tissue And How To Get Rid Of It?

We all have scar tissue and we all will develop more in our lifetime, but the majority of us do not understand how scar tissue develops in our body and how it affects us. As a North Vancouver Chiropractor, I reflect back at my time at Palmer West Chiropractic College in San Jose, California. It was in my first year that we had a pathology course and the subject of Scar tissue came up. We studied how the liver creates fibroblasts and lays down collagen fibers down to repair injured areas. To this day, I still explain the same details as I learned long ago. The importance of this subject is that today as Chiropractors we are able to help remove unwanted scar tissue from soft tissues. Two popular techniques come to mind, Active Release Technique and Graston Technique. I became certified in Active Release Upper Extremity towards the end of my Chiropractic program. It was there that I was amazed by the incredible results that this technique could do. Graston soon followed with surgical stainless steel tools that also removed scar tissue.

The results

The results from both techniques are incredible. As a Sports Chiropractor servicing in North Vancouver, I have worked on more than hundreds of shoulders that had rotator cuff problems. As a sports injury chiropractor, I would muscle test to find the specific damaged muscle. There are three prominent shoulder muscles within the rotator cuff group. These are the supraspinatus, infraspinatus and subscapularis muscle. Once I did several courses of the Active Release Technique the patient would see thirty percent increase in range of motion instantly. Over several treatments, the patient would ninety percent of the time regain full range of motion. This seems like a tall glass to swallow, but the fact is that clinically these were my results.

The Real Deal

Yes, scar tissue once removed allows the body to function properly again. Let us dive into how scar tissue develops on the body. Scar tissue forms after surgery, or injury to the body. According to the page Scar on Wikipedia the body will produce very similar collagen tissue as was there before, except the new tissue is less elastic and instead of having a cross fiber structure has parallel fibers. This seems to be the main reason why people who injure their calves, ankles, shoulders, back and other joints always find less mobility and recurring pain and injury that joint. This is because the new tissue surrounds the injury sight and lays down tightly to protect and heal the area. Scars also develop over months and years and becomes very dense. This new structure will block off proprioception and is much less elastic.

This is the cause of so many re injuries to an existing injury. The new joint or area is much stiffer due to its tensional stress and has less proprioception, which communicates where a joint is in time and space at all moments to the brain. There are also different problems that occur due to scar formation to different organs and structures but we will keep focus on the soft tissues that connect to our bones. This is why techniques such as Graston that has been around for centuries is known for its acronym IASTM, which is known as instrument assisted soft tissue mobilization. These tools have been around for a long time and used for removing scar tissue. By doing passes on the muscle, unwanted scar tissue is removed. By comparison Active Release Technique uses the practitioners thumb or hand pressure to remove scar tissue. This is done by putting the specific muscle or tissue in its contracted state, pinning down the muscle and then having the muscle go through active or passive range of motion. Because the bad tissue is trapped under the practitioners grip it is forced to break up during range of motion. It should also be noted that scar tissue forms as a result of trauma to the body. Trauma can be sitting on a chair causing stress to the pelvis and back or an auto mobile accident causing whiplash. During whiplash the anterior muscles on the cervical spine are torn and heal by excessive amounts of scar tissue. Subsequently, the cervical spine is pulled forward due to the scar tissue and the lordotic curve is lost and replaced with a military or reversed curve. Even worse, disc and bones will degenerate and symptoms will persist from the neck, shoulders and down the arms. Our bodies are always going through stress. However, when there is considerable trauma to the tissues major scar tissue formation occurs. Smaller injuries to the surface of the skin heal without much issues.

In conclusion to this blog about Scar tissue, it has come to my attention how important it is as a Chiropractor to educate everyone on the success of these incredible techniques. We have to remind each other that it is possible to recover from long standing injuries. But there must be a different course of treatment. The remedies of the past will not work.

Do you remember when your father used to tell you? “When I was your age I use to walk 3 miles in the snow to school and back home.” Well, I tell the new generation of children tall tales of children growing up without tablets, gaming systems and smart phones. Yes, it is true, people use to play outside with rocks and go biking for fun. It seems far-fetched from today’s world, which is exponentially evolving so quickly that we do not have time to reflect on the changes. As a child, I remember watching back to the future II and recall the scene where Michael J Fox was talking on a futuristic device that transmitted video conferencing with the telephone at the same time. The world is turning, we must turn with it!

As a North Vancouver Chiropractor that has travelled the world, grew up in Canada in the early eighties then moved to the United States and back I have seen a lot of changes so far. Social media is dominating our dinner tables and influencing our decisions. Imagine our kids and how they are developing due to this. Is it our responsibility to limit the amount of time they are playing games or on social media? Above all else, negative changes to a child’s behaviour may not be the only thing suffering. Imagine all the physical changes to their bodies going on.

Are electronics the new bad guy? If superman or batman were real would we be seeing them fight the violent video games that influence our children. Well for now they are not real and we must discern what is good for them ourselves.

Is Electronics the bad guy?

Changes in postures, repetitive traumas to their joints and lack of sleep are all causing negative health effects. What should be done about this? Should we remove all electronics from our kids? It seems very difficult to do so, when we are all immersed in social media and electronics. In order to stay competitive at work you must be up to date with the latest technology. Maybe moderation is the key to keeping a balance between reading, sports and electronics. Mind you as a Chiropractor I do indulge in X box one games once in a while with my step son. We enjoy playing EA NHL Hockey 2017, which I find to be a healthy alternative to the violent games such as Grand Theft Auto (GTA) and Call of Duty. Even so, a small amount of video game playing can cause physical problems still. Kids may suffer from the same problems adults do just in smaller doses.

It is hard to believe the world is changing so fast. Do we go against change or adapt with it. Only time will tell what the future brings.

Upper cross syndrome, which is classified by forward head posture and rounded shoulders is what most white collar workers suffer from. Endless hours working at a cubicle staring at the computer screen with minimal breaks. The symptoms that arise from upper cross syndrome are numbness/tingling in the hands, shoulder pain, mid back pain, neck pain and headaches. If the cause of the problem is not rectified then symptoms can cause long term arthritic changes to the body. Subsequently, degenerative arthritis will develop and cause long term disability. Much like the adult in the scenario just described, children playing games or electronics for hours on end can also develop poor posture that lead to symptoms.

We can help

Children are much more resilient to pain than adults because they are still growing and have high amounts of testosterone and growth hormone that repair the body quickly. Unfortunately, as we age the amount of growth hormone and testosterone secreted reduces. So even if children don’t feel the effects of bad posture and poor ergonomics, they might feel the effects later in their life. As they keep new static postures, their spines will change in posture. They might go from a healthy lordotic curve to a military curve or even a reversed curve in their neck. Why does this matter you might ask? Well imagine a ten-pound bowling ball centered on the top of your first vertebra. If the spine is in perfect balance, meaning the neck and low back curve facing opposite directions than the mid back constitute the same amount of bones as the neck and low back together. If the neck curve reverses and mimics the mid back curve this causes a complete disarray for the spine. The immediate and long term effects are degenerative disc disease, arthritis changes to the vertebrae and a host of symptoms such as numbness/tingling, headaches, reduced range of motion and pain.

Pregnancy is tough

Pregnancy can be a difficult process for the majority of women. Statistics show almost 50 percent of women suffer from back pain when pregnant. Nausea, pain and immobility can become the wrecking ball for anyone trying to battle themselves through a normal day. Pain can become quite normal during pregnancy as joints can start to shift due to the relaxin hormone. The relaxin hormone is produced by the ovary, breast and placenta. Furthermore, the relaxin hormone functions to increase cardiac flow, renal flow and helps relax pelvic ligaments. This is most apparent during the third trimester as the body must create more room in the uterus for the baby and for the birthing process. As the pelvis shifts pregnant women most commonly experience symptoms of sciatica due to the increased instability of the pelvis and surrounding joints. On average women gain 30lbs during their pregnancy.

Real life experience

There are some things that you can learn in a classroom, read about in text books or journal and see first with your own eyes. But practicing and treating patients first hand gives you a three-dimensional understanding of the practicality of the any condition. It has been my very own experience that Chiropractic care is extremely safe and effective for relieving the underlying issue of pain and sciatica. Best practices

The best techniques for treating pregnant patients in my own opinion as a North Vancouver Chiropractor are cox flexion decompression, and activator methods technique. My professional opinion may be shaped due to the fact that I am advanced certified in both techniques and have a lot of clinical experience with both. When a woman becomes pregnant they have to be careful not to endure any force to their body. Therefore, any technique that is used must be non-force. The practitioner should have a special table that accommodates large bellies, with either a lumbar drop or a special foam insert for the stomach. The lumbar drop in my opinion is extremely practical for treating pregnant patients.

As a population we need to be focused on how to aid females with postural pain during pregnancy. More technology has to be developed to help reduce pressure on joint and reduce stress on the uterus.

The woman’s belly can fit into the drop piece for the lumbar area and the rest of the patient’s chest, head, and legs can be supported properly. Subsequently, the female’s spine can be relaxed and in proper position. Thereafter, activator methods or cox flexion can be applied to the affected joint. Both techniques are completely different and can be applied together. Activator methods is a non-force tool that applies a safe adjustment to the joint without any chance of soreness. Cox flexion technique allows the joint to be stretched and a negative intradiscal pressure to be applied to the disc.

Alternative techniques

Other therapies that are safe and effective for pregnancy patients are TENS, massage, interferential therapy, and acupuncture. Knee problems and high number of knee surgeries

The number of knee surgeries is on a steep rise in the past two decades, almost doubling. There has been much discussion into why patients have been opting for surgeries earlier in life and why the number of surgeries has doubled. Some preliminary research has shown that the obesity epidemic and population growth has been the main contributor to these factors. But some researchers are still not convinced that these are the definitive causes.

Unfortunately, population growth and obesity statistics do not follow the same growth pattern. During a nine-year period between 1999 and 2008 knee surgeries in the USA more than doubled while population growth increased 11 percent and the number of people falling into obese category increased by roughly 20 percent.  Therefore, there must be other reasons for the increased number of knee surgeries. One explanation might be the increased injuries sustained by the population and the increased number of patients opting for surgery. Surgeries for the knee have become less invasive explaining to why patients might be opting for surgeries. Surgeries are quicker and healing times are faster.

We are educated on television programs that when your knee is in extreme pain it is time to get tough and persist through the discomfort. Instead of resting, take a pain pill and anti inflammatory to ignore you problem. Well this is completely the contrary. If you are in pain this is a sign to stop and rest. What until your body tells you it is okay to slowly get back into your favourite sport!

Baby Boomers

Opinions in the medical community are stating that the baby boomer generation is opting for knee surgeries as if they were going to get a coffee at Starbucks. Baby boomers are subjecting their knees to more high-risk activities and at the same time living much longer than their parents. A hundred or more years ago communities were much more involved in labor-intensive daily activities and were subjecting their knees to the same type of stress. However, unlike today the life expectancy was not as long. Several decades ago the leading cause of knee surgeries was rheumatoid arthritis, while today the most common cause has become osteoarthritis. Osteoarthritis is caused by mechanical stress to the knee joint.

In comparison, hip surgeries are on the mend. The reason for the decrease in hip surgeries is due to better screening at an earlier age by medical doctors. Screening for such things as hip dysplasia has dramatically improved. Unfortunately, knee surgeries do not have the same luck. Is the Ice in RICE out dated? Lymphatic Systems

Get the facts straight!

It may be time to sit down and hold on to your seat. Yes, there is something we need to talk about and it may shock you! Well, it is not going to be an, it’s me not you conversation. This talk is about the decades old rule about icing injuries. It may be time to stop icing as new information contradicts what we believed to be true. After an injury we may need to manage our healing a little differently. I wrote a blog on RICE, which is an acronym for rest, ice, compression and elevation. The blog was an excellent experience for me to write. I got to research many new thoughts on RICE and healing. There is a whole new school of thought regarding how to manage an acute injury.

ICING

The thought behind limiting ice during an injury also applies to compression and elevation. Before, it was thought that ice should be applied to limit the amount of swelling to an area. Inflammation was thought to be an overzealous event that may have caused too much scar tissue to be produced. The outcome would be too much scar tissue causing limited range of motion and continued pain. Subsequently, with improper healing without RICE re injury would be a high probability.

Lymphatic Systems

Our bodies have a system to deliver healing factors to the site of injury. The heart, artery and veins work with the lymphatic system to deliver and remove fluid to injury sites. Icing causes vasoconstriction of the circulatory system, but causes increased permeability of lymphatic tissue. The lymphatic system normally works as a scavenger system to remove access fluid and waste back into the circulatory system through lymphatic trunks near the neck. When ice is applied to the area of injury, permeability of lymphatic tissue in the area of injury increases and fluid reserves flow back into the site of injury.

Unfortunately the lymphatic system does not have a pump (heart), which works to circulate white blood cells and excess fluid back into the circulatory system. The lymphatic system works by contractions of the lymphatic vessels and skeletal muscle. Thus our mindset has to change regarding RICE. Not just about ice, but about compression and elevation.

Maybe the body is designed to perfectly remove waste and heal desired areas. Compression and elevation may reduce the flow of lymphatic tissue and blood flow to an area. What we may want is to use passive and active range of motion to help the lymphatic system work. We should be weary as new research on recovery and rehabilitation comes out all the time. But a little common sense goes a long way. It may be to our advantage to still use ice in moderation. Ice has analgesic properties and fewer side effects then taking OTC’s. Compression and elevation should also be used in moderation.

RICE What Does It Stand For?

Keep Calm and follow the Basics!

Do the same principles of RICE (Rest, Ice, Compression and Elevation) still hold true? With all the new technology and research there must be some magic therapy that can heal all acute and chronic injuries. There are so many different new therapies that are on the market for practitioners. But one thing will never change and that is that the basics are the basics. We need to adhere to the basic principles of healing. So what are the basics and why do they still have the best results. This is a great question and one that we should all follow. RICE, the acronym for rest, ice, compression and elevation has four different components that can be done for practically free but are extremely affective. We should begin by giving a general understanding of how injuries occur to the human body.

Wear and Tear

Our bodies are constantly breaking down due to wear and tear. We are aging from the moment we wake up to the time we go to sleep. Our entire body is constantly rebuilding itself. Then add to that work stress, sports activities, and home life. Every step we take we are putting load on our joints to support us, our organs to provide the oxygen to the tissues and our nervous system to manage all of these systems. Therefore, all of our bodies systems are susceptible to trauma. When our bodies are subjected to these stressors the result is inflammation to all parts of the body. When the arteries incur inflammation we develop atherosclerosis. When we have inflammation to the soft tissues we develop scar tissue. And finally, when we have chronic inflammation to the bones and discs we develop degenerative bone and disc disease.

What Is Chronic Pain

The number one indictor of inflammation is pain. People who suffer from chronic pain also suffer from constant underlying inflammation to their tissues or bones. X rays of a mid 40’s patient that has suffered from long standing chronic pain show vertebrae with jagged edges known as osteophytes, and decreased disc height. In radiology the most common patient to show with these radiographic changes are females over forty who are semi obese. The additional weight causes severe stress to the low back and degenerative changes begin to occur.

The healing process in our body can be excessive and cause diminishing returns. RICE is an amazing remedy to help heal acute or long-standing injuries. Resting is the first part of the healing process. By taking a break and not putting your body under further stress, your body will be able to focus on healing the injured area. Ice is an excellent natural analgesic to reduce pain. Ice has been known historically to reduce inflammation as well. But recently more research shows that ice should be limited for swelling and healing. Compression and elevation are the last two parts of the equation of RICE that help with swelling and inflammation. It should be noted that RICE is a powerful way to take a step back and rest.
